Health Benefits: Vision Plan
When you need to take care of your eyes, you have Benefit Options. The Avesis Vision Plan is a Preferred Provider Organization (PPO). Under the PPO, you have 3 options for eye care: standard lenses, contacts, or an allowance toward the cost of Lasik surgery.
Benefits vary depending on whether you seek services from a Member Doctor or a Non-Member Doctor. You may elect vision coverage for yourself, or yourself and your family. The employee pays the full cost of the monthly premium which is deducted on a pre-tax basis.
