If you have become a victim of identity theft, the Social Security Administration (SSA) has an internet web page that will provide you with very important information on what to do if someone else is misusing your Social Security Number (SSN).  The SSA also has publications available at http://www.ssa.gov/pubs/10064.html  on identity theft and instructions on when and how to contact the SSA.

The SSA will suggest that you contact the Federal Trade Commission (FTC) if someone is using your SSN:

When you contact the FTC, they will suggest you file a report with your local police department and advise you to get a copy of the report.  The Phoenix Police Department has good information, which can be accessed at Phoenix Police Department . Another FTC suggestion is for you to contact the fraud departments of any of the following three national credit reporting organizations:
