School and Instructor Certification Information

Attention Defensive Driving School Owners and Designated Principals:

School Renewal is now open.  

A letter regarding renewal was sent to all school owners/designated principals and included a copy of two applications: a school application to be completed by the designated principal, and a business member application to be completed by each business member.  You may use the applications included with the letter, however, they must be typewritten.  There is no online application submission.

The applications may be downloaded, filled out by typing the information into the application, printed, and mailed or delivered to the Supreme Court. Applications received or postmarked after October 1, 2015 will be assessed an additional $50.00 fee.

Please note: For each school that is renewing, please file a completed application (without reference to another school’s application or attachments) and include all attachments and other materials with respect to that application.

Applications will be accepted September 1st through October 31st, 2015. 

The renewal fee for applications received on or before October 1st is $50 and $100 for applications received October 2nd through October 31stAll fees are non-refundable.

Renewal applications must be filed online by midnight October 31st, or postmarked by October 31st in order for your certificate to remain active.

Congratulations on your decision to join the team of elite professionals which make up the Defensive Driving School organization in Arizona.

The following documentation provides information about starting an Arizona certified defensive driving school and/or becoming a certified defensive driving instructor.  Please read all materials before completing an application. The applications are form-fill documents which require you to open the documents, complete the forms online and print them out to send in to the Arizona Supreme Court, along with any required attachments.  You cannot submit these forms electronically.
