REMINDER
All Contractors:
The first Quarterly Core Performance Goal Report is due on or before October 31, 2012. Performance Goal Reports shall be submitted within thirty (30) days of the end of the previous quarter. One (1) form is required to be completed for each program/service, therefore Contractors may have to complete more than one (1) form based on the number of individual programs/services currently contracted.
The submission of this form is required even if the Contractor did not receive referrals during this quarter. Please mark the text fields as “0”.
The Contractor shall use the ASC/AOC Core Performance Goal Report form(s) to complete and submit the report for each service under contract. The ASC/AOC Core Performance Goal Report form(s) are accessible via the JJSD website at:

The Contractor must maintain a copy of the completed Core Performance Goal Report(s) for review under this contract. Prior to submitting the form, use the “Print Form” option at the end of the document to print your copy.
Please submit each completed form to the AOC by using the “Submit by Email” option located at the end of the form. If you do not use Outlook for your email account, select the “Internet Email” option, click “ok” and then select the “Submit by Email” option to send the form electronically to the AOC. The form must be sent electronically to AOC for data generation.
