The John R. Justice (JRJ) Student Loan Repayment Program Service Agreement requires beneficiaries of the program to remain employed as an eligible prosecutor or public defender for a period of service of not less than three years (36 months) unless voluntarily separated from employment.
The Bureau of Justice Assistance requires each JRJ Administering Agency to annually prepare a verification of JRJ beneficiaries who have either completed or have not completed their initial JRJ service obligation. As such, all individuals who have received a JRJ award as well as their employer must complete a Beneficiary Service Verification Form to certify the beneficiary remained in an eligible position during the service agreement period.
If you have been notified by the Administrative Office of the Courts JRJ grant management staff, please complete and return one of the following Beneficiary Service Verification forms, based on your initial term of service start date. In addition, please submit a completed employer beneficiary form. Please return/submit both completed forms together.

2. Initial Term of Service for FY15 through FY17 (10/1/2015 – 9/30/2018)

4. Secondary Term of Service for FY16 (10/1/2016 - 9/30/2017)
