Certificate of Service ARCP Rule 5(c)(2) Each document filed into a case must be served on the others involved in the case.  This form is to be used to indicate what service has taken place.
Consent for Electronic Distribution ARCAP Rule 4.2(h) Self-represented litigants wishing to receive electronic communication from the court may provide an email address and consent to its use using this form.
