Victim Notification

A crime victim has the right to request a copy of the court's decision on the appeal from the Court of Appeals or the Arizona Supreme Court at the same time that copies are sent to the prosecutor and the defendant.

If you are a crime victim and you know that the defendant has a case pending in the Division One of the Arizona Court of Appeals, Division Two of the Arizona Court of Appeals, or Arizona Supreme Court, fill out the appropriate Victim Request for Decision or Opinion form.

Your contact information will be kept confidential and used only to enable the Court to send you a copy of the decision.

After printing and completing the Victim Request for Decision or Opinion form, mail or deliver the form to:

The filing of this form in Division Two will also effect victim notification in the Arizona Supreme Court if that court ultimately issues a decision or opinion.
