Checklist for Request of Accommodated Testing
This checklist is designed to assist you in gathering information and preparing material to submit a complete request for accommodations. Use this checklist to complete all information listed below that pertains to the accommodation you are seeking. These forms must be used for accommodations requested for the February 2011 bar examination and all examinations administered thereafter.
Please note: If the Arizona uniform bar examination is administered as a remote exam, it will have a different structure in terms of the number of testing sessions, duration and breaks. Please contact [email protected] for more information.
STEP 1 - READ AND PREPARE REQUIRED MATERIALS

STEP 2 - COLLECT AND MAIL THE COMPLETED TEST ACCOMMODATIONS FORMS*
____ Completed Request for Testing Accommodation (FORM 1).
All applicants seeking test accommodations must submit Form 1.
All of the following as applicable (To be completed and mailed together with Form 1):

Physical Disability Verification Form (FORM 2)

• You will request that your physician or other professional licensed to diagnose and treat your disability complete the form and return it to you for submission to the Committee.

Learning Disabilities Verification Form (FORM 3)
|
|
• You will request that your physician or other professional licensed to diagnose and treat your disability complete the form and return it to you for submission to the Committee.
|
|
• Submit copies of your high school transcript, as well as your undergraduate, postgraduate, and law school transcripts. Additional documentation as outlined in the Guidelines will assist the Committee in properly assessing your request.

Attention Deficit/Hyperactivity Disorder (ADD/HD) Verification Form (FORM 4)

• You will request that your physician or other professional licensed to diagnose and treat your disability complete the form and return it to you for submission to the Committee.

• Submit copies of your high school transcript, as well as your undergraduate, postgraduate, and law school transcripts. Additional documentation as outlined in the Guidelines will assist the Committee in properly assessing your request.

Mental/Psychological Disability Verification Form (FORM 5)

• You will request that your physician or other professional licensed to diagnose and treat your disability complete the form and return it to you for submission to the Committee.

Law School Verification Form (FORM 6)
|
|
• You will request that the law school administrator or professor responsible for authorizing test accommodations complete the form, stating all test accommodations granted.

• You are required to submit this form if accommodations were granted in any law school testing. Please submit one Form 6 for each law school, if more than one.

Bar Admissions Administrator Verification Form (FORM 7)
|
|
• You are required to submit this form if accommodations were requested on a bar examination in another jurisdiction, whether they were granted or denied. Please submit one Form 7 for each jurisdiction, if more than one.
