The Arizona Traffic Ticket and Complaint (ATTC) form is a multi-agency form that affects the public, the courts, and law enforcement. Therefore, law enforcement agencies are strongly encouraged to work with their applicable court(s) prior to submitting changes to ATTC forms.
Each law enforcement agency or public body responsible for issuing ATTCs is required to submit any “substantial variations” to the ATTC form to the Supreme Court for approval (Rule 4, Rules of Court Procedure for Civil Traffic, Boating, Marijuana, and Parking and Standing Violations). The substantial variation examples below are not all-inclusive and may not address each agency’s particular situation.
Many AJACS courts and law enforcement agencies are moving in the direction of using electronic handheld devices to issue citations. If you are an AJACS court considering this option, please review the information below.
Format of the citation: The AOC has its own SmartPrint™ program that is available to the AJACS courts for free. A citation, which was developed with cooperation of the AOC, the courts and the vendor, is available in a template format. If your court does not wish to use the template format, you will need to purchase your own SmartPrint™ program and submit the citation for approval by the Supreme Court.
Approval is needed: You must submit the AOC’s SmartPrint™ citation for approval; however, the process should be fairly quick and simplified by using the template format that is currently provided. Your court may deviate from the template format, but any deviation in either the citation format or the Warning to the Defendant section, may result in longer approval times and increased money for programming by the court.

Approval is needed: All Photo Enforcement citations must be submitted to the Supreme Court for approval as the photo enforcement citation is substantially different from the ATTC form provided in the Arizona Rules of Court, and is considered a “substantial variation."

Approval is needed: Agencies must complete the Request to Approve a Substantial Variation to the ATTC form for consideration of any proposed “substantial variations” to the Arizona Traffic Ticket and Complaint (ATTC) form. A citation is considered substantially different, and will require approval by the Supreme Court, if it deviates from the ATTC form provided in the Arizona Rules of Court.
