Probate Forms

During the course of your service as a guardian, conservator, or personal representative, you will be required to file certain documents and reports with the court.  Some of the documents and reports may be required by your court only, while other documents and reports are required by all courts in the state.

Administrative Order No. 2012-65 issued 7/31/2012 adopting Arizona Code of Judicial Administration § 3-302; Probate Court Forms, specifies the required use of certain forms to bring uniformity and consistency to probate proceedings statewide. 


Orders and Acknowledgments & Proof of Restricted Account Forms

Forms 1 through 4 and Form 10 are considered preferred forms and may be adapted by deleting information that does not apply to a particular case or by adding other relevant information under certain conditions outlined in *ACJA § 3-302.

Conservator Account Forms

Forms 5 through 9 are considered exclusive forms and must be used in the format provided, except for good cause shown in a particular case and upon court order. 

Form 5 – Conservatorship Estate Budget

Form 6 – First Conservator’s Account 

Form 7 – Conservator’s Account

Form 8 – Final Conservator’s Account

Form 9 – Simplified Conservator’s Account

 *ACJA § 3-302, which requires the use of the standardized conservator account forms, is effective September 1, 2012New cases appointed on and after September 1st will be required to use the new forms immediately.  Conservators in existing cases will be required to use the new forms after the court approves their next annual account.
