Filing a Complaint

All current fiduciary licenses will expire on May 31, 2022. The Certification and Licensing Division uses an online application form. The Division will begin receiving renewal applications on April 1, 2022, through the Arizona Judicial Branch Certification & Licensing Portal at: Arizona Judicial Branch Certification & Licensing Portal 

A link to the application instructions is provided below. Please review these instructions thoroughly before beginning the online application process to ensure all required information and documentation is prepared before starting the online submission. You will need to fill out and upload two forms in the application: a Continuing Education Form which you will scan with your certificates of completion, and an Authorization and Release Form.  You should fill out, then scan the two documents and have them ready to upload in the online portal.


All individual certificate holders must register for portal access. Registration may be completed on or after April 1, 2022 by selecting “Login/Register” from the upper right side of the screen and “Don't have an account?  Register Here” from the login screen at  Registration requires an email address and social security number.  The email address and SSN used MUST MATCH that on file with the Division.  If your email address has changed or if you are uncertain which email is associated with your license, please contact the Division BEFORE you attempt to register in the portal.  If requesting a password, your email address is your “User Name.”


Once you have provided the registration information required and selected “Register,” you should receive an email with a link that will prompt you to set a password for your account. Please note that system emails might appear in junk or spam folders and will be from AZCourts.


To obtain information once in the portal, click the tabs on the grey/blue text menu across the middle of the main page.  After April 1, 2022, the renewal application may be accessed by clicking the “licenses” tab from the main page menu and selecting “renew” from the “actions” drop down menu to the far right of the license number information. Once an application is started, information concerning status can be viewed by selecting the “applications” tab from the blue text menu on the main page.  If you exit the portal prior to completing an application and wish to go back to where you left off, log in to the portal, click the “applications” tab and select “resume” from the “actions” drop down menu to the far right of the application record.

Once you complete and make payment, make sure you click SUBMIT.

For renewal applications completed before May 1, 2022, the registration fee is $400. Applications submitted on or after May 1, 2022 will be assessed an additional $50 fee.  If you are charged an erroneous fee of $450 prior to May 1, 2022, please call us and we will correct it.

Note: If you have submitted a timely and complete application for renewal, your existing license remains active until the administrative process for review of the renewal application has been completed.  If you do not submit a complete application before May 31, 2022, your license expires and you must stop providing licensed fiduciary services.
