Performance Goal & Diversity Reports

The AOC collects quarterly data from Contractors on the performance of services. Diversity reporting of both staff providing services and youth receiving services is collected annually. This data is used to complete the Performance Goal Report and the Diversity Report for each fiscal year.

Performance Goal & Diversity Reporting Requirements for Contractors

Performance Goal Reporting

Providers under contract with the AOC are required to submit quarterly performance goal reports in accordance with AOC Standard Terms and Conditions, Paragraph 61(g) for Full Procurement and Paragraph 49(f) for Independent Practitioners. Performance Goal reports shall be submitted within thirty (30) days of the end of the previous quarter. Please note one (1) form is required to be completed for each program/service, therefore Contractors may have to complete more than one (1) form based on the number of individual programs/services currently contracted. For example, Delinquency Prevention, Behavior Specific Services (service code 190) may have several different program types. Therefore, a separate form must be completed for each individual program type (i.e. drugs and alcohol, truancy, gangs, etc). Data reported must be specific to only youth serviced under the AOC contract. You may access the Performance Goal Reporting Form for each service by clicking on the service name below.

    Diversity Reporting

    ALL Contractors under contract with the Administrative Office of the Courts (AOC) are required to report staff and client diversity information to the AOC at the end of each fiscal year (June 30th). Diversity Reporting is specified in the AOC Standard Terms and Conditions, Paragraph 61(f) for Full Procurement and Paragraph 49(e) for Independent Practitioners. The Contractor, shall by August 15th of each year submit Diversity Reporting information for only the personnel providing direct care services for the AOC contract and clients served under AOC contract. The submission of the Diversity Reporting Form is required even if no referrals were received by the Contractor in the fiscal year.

    In accordance with Paragraph 36(m) of the AOC Standard Terms and Conditions for Full Procurement and Paragraph 34(i) for Independent Practitoners, the Contractor will provide to the AOC a list of direct services personnel, volunteers/interns on at minimal a yearly basis, and/or upon request or at the start of any contract year to include name, degree, license type and corresponding license number. Please submit the agency list of current direct service and volunteer/intern personnel along with the corresponding information requested to the AOC with the Diversity Report submission.
